A good way to track project problems is with a fish-bone diagram, like the one shown.
This fish-bone diagram lets you quickly and graphically show customers the source of any problems that crop up in your project. Each branch has a total number of errors, and those branches that are further down show more detail.
In this example, the "User Error" branch tells customers that there are seven problems, four of which could be eliminated by training, with the other three being technical glitches.
This helps you give your customers a 10,000-foot view of the project's major problems, and helps them understand what areas need to be worked on first.
